Older Persons “Have Your Say” Questionnaire

The Older Peoples Commissioner for Wales is providing a platform for our valued older residents to engage in the forming of future policy, provision and well-being of older persons. The questionnaire asks what could be improved, how safe you feel, whether you are recognised and treated fairly and importantly, are you listened to?

The questionnaire (attached) can be edited and returned by email, printed and returned by post or you can speak to a real person who will help you through the process.

I have highlighted (yellow), in the attached document, the details / options for returning the form. If I can help further, I am always in the office between 1000-1400. So call in, phone or email me and I will get it sorted out for you. Sometimes the additional information is tricky on a computer so get in touch and I will give you some pointers.
